Cut off the hardened tip and you can usually save most of the caulk inside.

Start by holding the tube upright with the nozzle pointing up so any loose skin can fall away, then use a utility knife or heavy scissors to cut the nozzle off one quarter to three eighths of an inch from the base depending on how much blockage there is. If the caulk is very tacky near the cut you can gently press from the back to extrude a small bead and wipe it with a rag to check consistency before attaching anything. Fit a replacement nozzle such as a Tubanu tip or a screw-on replacement nozzle that matches the tube thread, making sure the new tip seats evenly so it will not leak at the base. For tubes without compatible threads you can stretch a short length of heavy duty tubing over the cut end and tape it tight with waterproof tape to form a temporary nozzle. After each use push a nail or a piece of stiff wire through the new nozzle to prevent reblocking, and wipe the tip clean before capping. Store the tube standing on its cap in a cool dry place and consider transferring the remaining material into a caulk saver or an empty sausage packer if you expect long term storage.

People often cut too little and leave a partially blocked nozzle which causes uneven beads, so cut back to fresh material and verify flow before use. Another common mistake is forcing a mismatched screw-on nozzle which will leak at the joint, so use a short sleeve of tubing and tape for a snug transition or buy the correct replacement nozzle size. A third issue is failing to clear the tip after use then sealing it poorly, so always push a wire through, wipe the tip, and cap with a fitted cap or a caulk saver to keep air out.
